The Rail Accident Investigation Branch (RAIB) has released its report into a fatal accident that occurred at Ickenham Station on 28 March 2025.
The tragic accident happened at around 10:30 pm at the Ickenham London Underground station, where a passenger fell onto the tracks and suffered fatal injuries as an oncoming train arrived at the station.
The Rail Accident Investigation Branch has concluded that the passenger had lost their balance on the platform, and station staff were unaware that the passenger had entered the station in a vulnerable state and fallen onto the track.
The brighter platform, contrasting with the darker tones of the track bed, may have obscured the train operator’s ability to see any obstruction, and the under-platform recess was housing cables at the time.
CCTV has been inspected to allow investigators a clear idea of the events that took place leading up to and during the accident.
Going forward, the Rail Accident Investigation Branch has provided three recommendations to be addressed across the London Underground.
To ensure that under-platform recesses are accessible, to implement site-specific risk assessments at stations, and to provide staff with appropriate training to safeguard passenger safety.
